Ventura County 4-H 2019-20 Re-enrollment, New Enrollment, & Adult Volunteer Online Training Now Open

 

This year the enrollment process is paperless, however, our club does need a copy of the “Treatment Authorization & Health History” a two-sided form to be completed, signed, and turned in with your enrollment fees.  Click here to complete a printable: 4-H Member Treatment Authorization and Health History Form. Please print this out and mail to Nancy Blomquist with enrollment fees. 

Enrollment fees for the 2019-20 year are:

  • Youth = $82.00 (fee due for the first child enrolled in your family)
 
  • Siblings = $77.00 (fee due for each additional child in family)

HOW TO ENROLL IN 4-H for 2019-20:

To re-enroll  youth members: 

·         Go to: 4HOnline

·         Use the e-mail address and password that you already established when you created your family profile

·         Do not create a new family profile and do not create a new profile for youth who are already in the system

·         Follow the prompts to ‘re-enroll’ each returning adult and youth

·         Mail Treatment Authorization & Health History Form(s) and fees to Nancy Blomquist.

To add a new member to an already established 4-H family:

·         Go to: 4HOnline

·         Use the e-mail address and password that you already established when you created your family profile

·         Do not create a new family profile and do not create a new profile for youth who are already in the system

·         Follow the prompts in 4hOnline to ‘add’ a new youth or adult member

·         Mail Treatment Authorization & Health History Form(s) and fees to Nancy Blomquist.

New Families/new members joining a club for the first time:

·         go to: 4HOnline

·         Create a new family profile in 4hOnline and follow the prompts to add a new youth member

·         Mail Treatment Authorization & Health History Form(s) and fees to Nancy Blomquist.

ADULT VOLUNTEER LEADERS:

RETURNING Adult Volunteer Leaders Online Training:

·         Re-enrolling adult volunteers will receive a confirmation e-mail upon completing the 4hOnline enrollment with instructions                  and links to complete the annual online training
·         Or go to 2019-20 California Returning Volunteers Training
·         Do not create a new account
·         Login using the e-mail address and password that you established for this training last time
·         You will be asked for an ‘enrollment key’. It is:  Ventura
·         Click on the ‘4-H & Youth’ box
·         Then click on ‘2019-20 California Returning Volunteers Training and follow the prompts to complete training
·       Save or print a copy of the completion of training certificate for your records.  It is handy to have if a question of 
        completion is raised by the county or state 4-H. 

NEW Adult Volunteer Leaders Registration and Online Training:

  • Complete the4-H Volunteer Interest Survey
  • Create an adult volunteer profile (or add a new adult to an already existing family account) on 4HOnline. You are registering as an adult (not a youth member) in the 4-H system. 
  • Complete a California Department of Justice Live Scan background check:
  • Use only this form. Do not use the forms that the Live Scan
  •  
  • operators provide. Print this one and bring with it you to the Live Scan Operator that you choose. Live Scan Form
  • New Volunteer Online Training. The instructions for this are automatically sent upon completion of your 4HOnline enrollment. If you have misplaced this information you should go to 2019-20 California New Volunteer Training.  The ‘key’ they ask for at the beginning of the training is:  Ventura

   ·         New Enrollment starts in August and needs completed by October.

 

Thank you for your immediate attention to the re-enrollment/new enrollment process. Enrollments are always a challenge and we are appreciative of our club members helping to make enrollments as smooth as possible for the Club Leaders, Fair, and the County Office.
